1. Application Form duly filled in by the Candidate along
with photograph.
Duly attested by the HOD of the
Institute/ College/ University where
admission obtained.
2. Copy of Graduation/12 Class pass certificate, as the case
may be.
Duly attested.
3. Copy of ST Category certificate issued by the competent
authority.
4. Copy of Domicile Certificate issued by the competent
authority.
5. Copy of BPL/AAY, Income Certificate and PHH issued by
the competent authority.
6. Bonafide certificate from the Head of the Institute/
College/University where admission obtained.
attested by the concerned Institute.
7. Copy of Aadhaar Card.
8. 4 No. Passport size Photographs.
9. Required undertaking on Affidavit in original. Duly attested by the Ist Class
Magistrate.
10. Copy of Fee receipt of admission in the Institute/
College/ University.
Duly attested.
11. Copy of PHH E-Ticket (16 digit) / Ration Card. Duly authenticated by the
Authorized Officer (AD/TSO) of
FCS&CA Dept.
12. Unemployed Certificate in original issued by the
competent authority.
